√ Yes! PT does not accept any health insurance, no private or government-sponsored plans, including Medicare. If Medicare is your primary insurance we will not be able to see you, even if you want to pay out-of-pocket. This is due to Medicare’s current rules, which negatively impact the private practice of health care.
Fees are due at the time of service.
You can pay with cash, check, or credit card.
√ If you plan to use your insurance company’s out-of-network benefit to partially reimburse the cost of Yes! care, you should check with your insurance company to see if they require you to have a physician’s prescription for physical therapy before you start.
If required, an “Eval and Treat” prescription is best.
√ Before your first visit, we need to know about your past medical history, including any surgeries, and your current medications, if any.
√ Organizing your information before your first visit will speed things along and give us more time to work together. Yes! PT will protect the privacy of your personal information to the fullest extent allowed by law, in compliance with HIPAA regulations.
√ You can print and fill out the forms at home and bring them with you or send them to us via our secure contact form.
You can also fill out forms in the office. If you choose this option, please plan to arrive 15 minutes before your scheduled appointment.
√ You should wear comfortable clothes (shorts, sweats, etc.), or you can bring a change of clothes with you, so that you will be able to move freely.
